Assessment & Determination of Priority Within the Fraser Health Authority
Admission to all residential facilities in our Health Region is coordinated through the Fraser Health Authority. Clients are assigned priority status according to urgency of need for placement. Clients occupying hospital beds, and clients considered to be in an emergency situation in the community, are given first priority for admission. Clients are also admitted to facilities on a “first available bed” basis, meaning that clients who are given priority are admitted to an available bed regardless of whether it is in a facility of choice. Admission to a facility of choice may still be requested, however the waiting period could be prolonged.
Facilities provide either Intermediate Care or Complex Care, or a combination of the two. The care level of a client is established by staff of the Fraser Health Authority. In an acute care hospital, this is done by the Client Care Coordinator, in the community it is done by the Case Manager, and in a facility it is done by the Facility Liaison. Admission Process at St. Michael's Centre
When a facility has a bed vacancy, it is reported to the local office of the Fraser Health Authority. The Health Authority informs the facility which client should be admitted. At St. Michael’s Centre, it is the Social Worker who then contacts the client (next-of-kin) to arrange admission and explain the procedures involved.
During the pre-admission interview with the client and/or family, the Social Worker will:
Review the St. Michael’s Centre Information Brochure
Ask you to complete the “Pre-Admission Database – Resident Information” form
Review and ask you to sign a “Residency Agreement” form
Discuss your assessed per diem rate and ask you to sign a “Pre-authorized Bank Debit” form
Review the resident’s current medical conditions and any special needs
Ask you to sign consent forms regarding the following optional on-site medical services: Dental, Podiatry, Annual Influenza Vaccination.
The client’s family physician is required to complete the following forms prior to the client being admitted to a long term care facility:
“Continuing Care Facility Admission Medical Database”
Form stating whether or not the physician wants to continue looking after the client once admission takes place.
These forms will be faxed to the family physician from St. Michael's Centre as part of the pre-admission process.
St. Michael's Centre has a list of physicians who are approved to practice at the facility. If the family physician is already on our list, he/she can automatically begin seeing the resident at St. Michael's Centre. If the physician is not on our list, and wants to care for the resident at St. Michael's Centre, an application to have privileges must be submitted to our Medical Director (a form for this will be given along with the other forms). If the physician does not wish to continue to care for the person at St. Michael's Centre, the new resident or a family member will be asked to choose one from our approved list. If this is not done by the time of admission, a physician from the list (provided they are available to take on new patients) will be assigned to be the resident's physician. Note: A resident cannot be admitted without an attending physician.
The physician who will be looking after the resident will also be asked to complete the following forms:
Physical Examination & Current Problem List
General Orders Protocol (standing orders for routine treatments)
Degree of Intervention (states the level of intervention to be used in case of sudden collapse or terminal illness)
If the person's current physician will continue to provide care at St. Michael's Centre, forms for these three requirements will be sent with the other Medical Admission forms. If a new physician has been assigned, the forms will be completed at the first visit post admission.
